Oxford Handbook of Tropical Medicine
Two special challenges of working in the rural tropics A fundamental requirement of good medical practice is the ability to communicate with patients. Western doctors working in tropical developing countries may encounter barriers to understanding their patients’ complaints and medical needs that far exceed simple linguistic translation. I fi rst encountered this problem when starting work in northern Nigeria in the 1970s. Hausa was not a diffi cult language (certainly compared to Amharic, with which I had struggled in Ethiopia) and, in any case, professional interpreters were available in Ahmadu Bello University Hospital. The real problem was trying to understand what the expression of a physical symptom implied in the local culture. Luckily, an experienced anthropologist, Murray Last, was living and working with a local Muguzawa community, 2 Hausa-speaking colleagues in the University, Allan Darrah a sociologist and John Froude a physician, produced an invaluable booklet ‘Hausa Medicine for Western Doctors’ and I had long conversations with a wise old friend in the Emir’s court, Madauchi Zazzau. With this considerable help I was able to begin to understand what an ostensibly straightforward symptom, such as feeling cold, might mean and might usefully be interpreted in the process of seeking a diagnosis.

Bonney’s Gynaecological Surgery
The infl uence of Victor Bonney and his pupils upon gynaecological surgery has developed from the publication of the fi rst edition of A Textbook of Gynaecological Surgery in 1911. The fi rst to the fourth editions were the results of the collaboration of Bonney with Sir Comynus Berkeley. Following the death of the latter, Victor Bonney produced the fi fth and sixth editions alone. Bonney ’ s pupils Howkins and Macleod then produced the seventh edition. The death of Macleod signalled virtually the end of those practising surgeons who had been trained by Victor Bonney. The very successful eighth edition was prepared by John Howkins and Sir John Stallworthy. These two great fi gures of Commonwealth Gynaecology had worked together as junior colleagues during the last years of Bonney ’ s clinical career.

BARR’S The Human Nervous System
Murray Llewellyn Barr (1908 to 1995) obtained his medical degree in 1933 from the University of Western Ontario in London, Canada, and after a few years in practice, he entered the Department of Anatomy at the same institution. He studied and taught neuroanatomy there until 1978. This period of service was interrupted by the Second World War, when he served in the Medical Branch of the Royal Canadian Air Force. In 1949, the direction of Barr’s research changed abruptly from neurohistology to cytogenetics. With Ewart G. (“Mike”) Bertram, who was then a graduate student, he had observed an intranuclear inclusion in neurons of female animals. This was the sex chromatin, now widely known as the Barr body; its discovery was an early landmark in the science of human cytogenetics. For this and his later work in the field, Murray Barr received more than 30 awards and honors, including the Kennedy Foundation International Award in Mental Retardation, Fellowship of the Royal Society of London, the Order of Canada, and 7 honorary doctorates.

Balancing Act : The New Medical Ethics of Medicine's New Economics Clinical
media or have become public through court hearings, all identifiable information used in case examples has been changed, and the structure of the case examples has been altered, so as not to identify the individuals involved. Instead, case examples have been constructed out of numerous clinical experiences so as to illustrate a general problem in bioethics, health care delivery or health care policy. Any similarity to actual individuals living or dead is purely coincidental. Also, though legal cases and principles are examined in this volume, there is no intention to provide legal advice. For legal advice there is no substitute for directly subvening the legal profession.

Balancing Act
With deep gratitude I acknowledge the colleague-friends who have read portions of this manuscript and shared their valued reflections. They have helped me enormously. They include: Robert Arnold, M.D., Howard Brody, M.D., Ph.D., Charles Dougherty, Ph.D., Howard Entman, M.D., Peter Jacobson, J.D., Erich Loewy, M.D., Thomas Kennedy, Ph.D., Laurence McCullough, Ph.D., Charles Pinches, Ph.D., Michael Rie, M.D., Lawrence Schneiderman, M.D., Margery Shaw, M.D., J.D. I would particularly like to thank those who have critically reviewed the manuscript in its entirety: Terrence Ackerman, Ph.D., H. Tristram Engelhardt, Jr., Ph.D., M.D., Leonard Fleck, Ph.D., Paul Menzel, Ph.D., Robert M. Nelson, M.D., Stuart Spicker, Ph.D., Stephen Wear, Ph.D., and several anonymous reviewers. The time, thought, and help they each so generously gave improved this volume immeasurably. While I therefore owe them great credit for its strengths, the book's flaws remain, of course, my own. And finally, I offer a special and very personal thanks to Tris Engelhardt. He has supported and encouraged my work since I was a graduate student - before I entered this profession, and long before he proposed (in a pub in Sydney, Australia) that I write this volume. Thank you, Tris.

Atlas of Neuroanatomy and Neurophysiology
This selection of the art of Dr. Frank H. Netter on neuroanatomy and neurophysiology is drawn from the Atlas of Human Anatomy and Netter’s Atlas of Human Physiology. Viewing these pictures again prompts reflection on Dr. Netter’s work and his roles as physician and artist. Frank H. Netter was born in 1906 in New York City. He pursued his artistic muse at the Sorbonne, the Art Student’s League, and the National Academy of Design before entering medical school at New York University, where he received his M.D. degree in 1931. During his student years, Dr. Netter’s notebook sketches attracted the attention of the medical faculty and other physicians, allowing him to augment his income by illustrating articles and textbooks. He continued illustrating as a sideline after establishing a surgical practice in 1933, but ultimately opted to give up his practice in favor of a full-time commitment to art. After service in the United States Army during the Second World War, Dr. Netter began his long collaboration with the CIBA Pharmaceutical Company (now Novartis Pharmaceuticals).
